1. Download the prescribed Form-A and Annexure-I
2. Fill the application completely by hand and sign in full
3. Attach self-attested copies of all required certificates in the prescribed sequence
4. Submit a separate application for each post, mentioning the post name clearly
5. Attach character certificates from two Gazetted Officers/Medical Practitioner/Sarpanch
6. Attach three self-addressed envelopes with Rs. 60 postage stamp each
